Appointment form
 

Here are the steps I do:
Tools
Forms
Design a form
I select the appointment form
the appointment form comes up in design mode with 10 tabs (appointment, P.2,
P.3, P.4, P.5, P.6, scheduling, all fields,properties and actions.
I want to change the informaion on the first tab that says appointments. I
am unable to do that

You can't change that page. You can try to replicate it and hide the
original but that's one of the pages in Outlook items that can't be changed.
