New Form
 
I designed a new form and then closed it. Outlook 2007 asked if I
wanted to save it and I clicked yes. But Outlook didn't prompt me for
a name. I have no idea where to look to find this form. It was created
in the Tasks folder. Any help will be appreciated.

New Form
 
What Outlook saved as a new task that has form elements now embedded in it. You can look in your Tasks folder for that item.

To use forms that you create, you need to publish them, not save them. See http://www.outlookcode.com/article.aspx?id=34
