Can't Find Custom Form
 

My head aches!! I created a custom form and called it MyForm.(clever huh?). I
published it as a contact form and now when I go to choose a form and select
the
contacts folder I don't see my form. Have no idea where to look, when or how.

Maybe you published it somewhere other than your contacts folder? The Personal Forms library would be a good place to look.

Or maybe you checked the "use only for responses" box on the (Properties) page, in which case you won't see it in the Choose Form dialog but would in the Design Form dialog.
