Outlook Form/Script

Hello everyone,
Im hoping someone on this forum can help me out. Ive had a lot of experience working with Excel, Vbscripts, Access, and HTML, but don’t spend much time in Outlook. Let me try to explain what Im trying to do…
I want to make a form that looks similar to this, not sure if it can be done in Outlook or what….

Then I want Outlook to take the data from my fields and send it in a format like this

Anyways, if anyone has any ideas for me, or if its simply just not possible, let me know.

Thanks for you help
