"Send mail items" IS checked, if that's what you mean.

Some more info:

1) ALL emails are ignored and not sent when offline, even small ones
2) I disabled the anti-virus (Avast), but that didn't help
3) I can delete the emails from the Outbox without a...

I have Outlook 2007 installed on Vista, and access mail on my Exchange 2K3
server.

When I am online, a sent email gets sent immediately - I see it for a second
in the Outbox - and then it is...
