Outlook Contacts always suggests "Custom Form Cannot Be Opened"

Dear All,

I have ascertained by searching "Message Class Field" that all contacts in
the search come up as found.
Therefore I think somehow I have put in place a Custom Form and would like
Outlook...

I have followed your suggestion and the search finds "exactly all" the
Contacts in the Folder.

Reverting to Custom Outlook 2003 Contact Form

Somehow I have managed to create a Custom Form and when I open a Contact I
get a message that Form cannot be read and will be opened in Outlook. I
recall a message about a change to a .dot template...
