Printing Contact List-Title for Address does not print in Outl

I was printing within Contacts - Card Style.

In Outlook 2007, when printing Contact List, the title for the addresses does
not print. It printed in Outlook 2003 and the titles for phone numbers,

Reinstalling Outlook 2003 inbox backup

Thought I'd followed all the instructions I found here for backing up my
inbox and contacts. I exported both files to a backup folder using the
"import/export" function under the "File" menu....
