Default How do I set the calendar form in 2003?

I've seem to have "lost" my calendar form. While in the calendar, I selected
a 'find', entered some data and received a list of appointments. Then I hit
"clear", which gave me all 4700 entries. Then I started to try to find the
forms that actually display the calendar instead of a list - to no avail.

Any help out there?
