I am trying to set up a recurring appointment for a specific day. This
recurrance has to show up for the 1st, 3rd and last out of the month for that
specific day. Does anyone know how to do this?

You will need to create 3 separate appointments, one for each day, and then
set the recurrence. Alternatively, you would need to use a third party
application such as WS Repeat Appointment available from Websetters.
