how do I change the default from 30mins to 1hr?

Set the Customize Current View, Other settings, Time scale to 60 minutes.

Default Default Meeting Length

I have set Customize Current View, Other settings, Time scale to 60 minutes, but new appointments still default to 30 minutes.

Is there anything else that needs to be set?

(Outlook 2003)

Default Default Meeting Length

If you create a new appointment by clicking the new button when in the month
view, it defaults to 30 min, if you use the day or week view, it uses your
setting.

You must be in a view with a time scale for the time scale setting to affect
the default duration.

Default default meeting length

You don't need to go into the customize view dialog - just right click on
the time scale in day or week view and choose a new time scale.
