Default Why would the OK button not be available under map custom fields?

I am trying to import a csv file to an Outlook calendar. I have done this in
the past with no problems, but now the OK button is not available under the
map custom fields. The import does not work - as you would expect. Any
suggestions?

Are you mapping the fields? If none are linked to the corresponding field in
outlook the ok button won't be visible.
