Default when using "Reply to All" one contact disappears

Thanks, I assume that Dan got the original message the same as you got it,
that would indicate that his email address was there - if he did not then
Michael's suggestion will hopefully shed some light.

There is a setting to "Automatically put people I reply to in my address
book" and maybe you could try changing that just in case your address book
is corrupt and causing the trouble. I have seen corruption where there is a
malformed entry but it is not visible, so you cannot delete it you have to
export, delete or preferably rename the old book and then import into a new
one.
