Default message sent multiple times

We're using OE 6 in the office. One of my coworkers has such problem:
sometimes the same message was sent multiple times (up to more than 10 times)
every few minutes. He is sure that he only sent once.

My other coworkers don't have such problem.

Thanks a lot.

June
