Default Spam penetrating filters

I have an email forwarding from my ISP to my main email address which I
always access first via Yahoo webmail on the internet before opening my
valid mail via Outlook Express.
Yahoo has a spam filter which has proved very satisfactory in general.
However every so often stuff gets through that and into my Yahoo in-box.
Fortunately I can read it there if I wish and if necessary invoke the
facility to shift mail from the inbox to the spam box which I invariably
empty very regularly.

Some time ago I began to notice that spam was by passing not only the Yahoo
spam filter, but the Yahoo site altogether and was getting directly into my
Outlook Express in-box. So I have also created message rules to delete or
relocate some messages for example those with the obvious obnoxious words in
their subject line or the like.

In spite of that I am receiving regular offers to "enlarge my apparatus
etc!!" in more than one of my Outlook Express accounts.
I just don't understand how this is being achieved ? Can anyone tell me how
these insidious emails manage to get past the various nets I have cast, and
more importantly what to do about it.
