Custom Form Templates

I would like to venture into creating a custom form. A very simple
task that has a present list of sections in the Notes section to allow
for the task to be routed accross our organization so different people
can make notes and "sign off" on the different steps of the task. Is
there a place where I could find a template for a form that could be a
good starting place for me?
