You should run the Inbox Repair Tool periodically and back up your PST file

Generally no, but it might depend on your version of Outlook as to whether

Then no, provided you are using the 2003 default data file, and not an
earlier version data file ported over from a previous version of Outlook. BTW Compacting probably wont show any size reduction unless you have removed a substantial amount of data, so dont go overboard with Compacting.
