Sorry to report this, but you can't. The only thing you can do is educate
the user to save the attachment where they want it first and then open it
from there.

Hi, Amanda.

I came upon this post when searching for a similar resolution. I wanted to
change the default location when saving an attachment directly out of an
email. Although you would still need to educate your user to first save the
attahment (before opening it), this method would save them a few clicks:
