Default Spouses with Outlook Contacts

I would like to improve how I have my Outlook Contacts. I use outlook for
everything, so my personal (family's) contact information is stored in
Outlook 2003. I also do a far amount of mail-merge printing so would like to
improve how outlook behaves.

We have many married friends and am upset with the basic features that
Outlook has for spouses. From what I can tell, all I can enter is a spouse's
name.

Is there any way I can store more than that? For instance, if I have a
friends that are both Drs. Is there any way I can enter a "spouse title" as
well? Or a spouse birthday, and so on...

You can use separate contact items for husbands and wives but you cannot do
what you want on one contact item.

I guess I'm not clear on the sort of thing I would like to do. Suppose that
I would like to send out Christmas cards and would like to use MS Word "mail
merge" to print my envelopes.

For the first line of the address, I might want "Mr. & Mrs. John and Jane
Doe." How does Outlook know to use Mr. & Mrs. (since I never imput Mrs. into
the contact item if Mr. John Doe is the contact and Jane is the "spouse")?
What if the 2nd person is a "Dr.", a "Mr." or whatever?

I think the nearest you can do with a single Contact and 2 people's details
on it is to merge using the fields like this :

Dear first name and spouse.

Labels could be a challenge as they may not even have the same surname.
Firstname and Spouse Surname would be fine for John and Judy Gleeson but not
if I had a different surname to my husband
