Default Contact Form Suddenly Changed for me too!

Sorry, I meant to say that I found the above topic but noticed there
was no resolution. I hope this has happened to someone else and can let
me know what to do to restore it back to default... I have screenshots
waiting if anybody can help. What I find interesting, is that when I
look at TOOLS FORMS DESIGN THIS FORM - it looks like the way it's
suppose to. with the familiar "Full Name" button and etc...

That link doesn't tell us anything about what changes you're seeing or in what version of Outlookk.

- come to think of it, I also just recently had to change my settings
to view Chinese characters. HOWEVER, the workstation next to me also
had to do this, but is not having this problem.... Hmmm...
please.help.somebody.anybody?!?
