Default I cannot see the Last(G) and First(M) fields in my contact form

I cannot see the Last(G) and First(M) fields in my contact form
Now, I can only see the "Full Name" field in my contact form
can anybody help ?

Default I cannot see the Last(G) and First(M) fields in my contact form

Click on the button "Name.." to see each part of the field "Full Name".
