Default Installing a custom form in Outlook Contacts 2007

I need help installing a cutom Contact form in Outlook 2007.
Also, when doing mail-merge with Word 2007 custom fields do not show up in
the available fields for selection, nor do the standard Outlook Contact "User
defined 1" 2, 3, and 4 fields.

Installing a custom from what source?

You need to start the merge from Outlook, not Word.

FYI, there is a newsgroup specifically for Outlook forms issues "down the hall" at microsoft.public.outlook.program_forms or, via web interface, at http://www.microsoft.com/office/comm...rogram_f orms
