Default LATIN AMERICAN NAMING CONVENTION - LAST NAME followed by mother's

How can I modify Outlook 2003 contact entry so that I can enter:

i.e. Juan Mateo VALDEZ Moreno

(First Name Middle Name Last Name Mother's Family Name)

So that when I print letters etc, I will get:

Dear Mr. Valdez,

rather than:

Dear Mr. Moreno

Thanks for your indulgence and I look forward to being enlightened.
