Default add a field like "anniversary" in contact form

Hi all,
I need to add a data field in the first page of the contact form.
How can I add a data picker?
How can I add this date to my calendar as a reminder??? (like the
anniversary does, but just once, not every year).

Thanks Brian, I already looked into there but without any goal.
Here http://www.outlookcode.com/d/datepick.htm it says:
1. Add the control to the forms design Toolbox.

Which control??? In the upper link On the "Microsoft Date and Time
Picker" I can't get anything to download/install...where am I wrong???

Why does using that field on the Details tab of a contact not work for you??

Well, it does work, but it doesn't do what I want.
First of all I need 4 calendar in the main contact page (to remind
when I've colled that person) and
second, one of them has to be a riminder...i mean if I set 1st
february 2007, it has to put an appointment in the calendar only in
that day and not for every year like the anniversary field does.
I hope to have been clear enough.
