How do I get a .csv file to import into a custom Contact Form?

Default How do I get a .csv file to import into a custom Contact Form?

I have designed a Custom Contact Form for a certain Contact folder and I have
assigned that Form to both the folder and subfolder. When I open up a
contact when in that folder, it does open up the new Custom Folder. The
problem is that I am trying to import a csv file into the Custom Form but
Outlook 2003 is defaulting to the standard Contact form and so is deleting
out precious data that I want to import.
