Default Address field is not visible in Address Card View

I created a custom contact form for my business. At some point in editting
the form I have changed the properties of the form so that the address' of my
customers do not show when in "Address Card" view.
If I click the address button, and complete the address in the pop up, the
address shows.
I have gone back into the form and tried to fix what I thought were
theissues to no avail.
