Default Employee Custom Form

I need to create an outlook custom form for our employees, and need to insert
the photo of each employee within. However, when I try to redesign the
General page of the contacts form, the page will change and I will loose the
original veiw. Is there a way to keep this control (insert picture of the
contact or employee) in a redesigned page, and what to do to add this control
in a new page?

Thank you for your help
