Default Contacts folder does not show address book

When I go to Properties-Outlook Address Book, the check box for "Show this
folder as an email address book" is greyed out. Is there a fix for this?

Of course.
Did you expect us to answer with no information?
Provide all relevant details including Outlook version and the steps you
used to corrupt your profile.

I'm using Outlook 2002 under XP. I don't know how the file was corrupted. When I opened Outlook yesterday, there was no info - no email, no contacts. I used "File - Open - Outlook Data File" to open the most recent outlook.pst file and apparently got back all the information except for the address book.

What do you mean? There is no address book in Outlook. If your Contacts are
there you have all your data.

That's interesting, since Outlook Help has a bunch of information about Address Books. What I was really hoping to resolve is that when I click on "To:" in a new message, there is no information at all in the "Select Names" dialog box. Also, when I type a name or a group or use "Check Names" there is no information. Is there an index file that needs to be rebuilt?

Default Contacts folder does not show address book

Not an index file. Just the view of your Contacts Folder that needs to be
configured. Those instructions are posted here daily:
