Default outlook form

I have a custom form with check boxes, txt boxes etc... is it possible to
have those fields set to read only after it has been filled and sent..so that
way the recipient can't change those fields? and if it is possible how would
one accomplish that?

The usual way to do that is to have two layouts, as message forms do by default, a read layout and a compose layout with the controls with the settings appropriate for each. Click the Edit Read Layout button in design mode to get started.
