Default USING CUSTOMISED FORM IN OUTLOOK

hi i have developed a customised form with some additional details to be put
like driving licence no. etc. i have already 1000 contacts in my contact book
now if i want to make the customised form as default form so that in the old
contacts also i can open in the customised form and just type driving
licence no.

any body can tell me how to do this?
