Default Maintain form when recipient forwards to another recipient

How do I keep the form when the recipient I sent the form to forwards or
replies - would like the form with the data to continue and perhaps be added
to. Currently, as soon as the recipient replies or forwards - the form
disappears.

Publish the form to the Organizational Forms library and on the Actions page in design mode, make sure that custom form is set as the form to be used by the Forward and Reply actions.

Where is the Organizational Forms library located? When I try to post to it
- Outlook throws an error.

The Organizational Forms library is a system folder on the Exchange server. The administrator must create it if it does not already exist.
