Default New form doesn't work from a file system - Precision

Precision

I defined and published a new outlook email form.

When I call this form from Outlook - File - New - Choose a Form -
Library - ... , the code in the form works.

When I call this form from Outlook - File - New - Choose a Form -
Template in file System, the code in the form doesn't work.

Any idea ?
