Default Add Page

I am trying to edit my contact form to have an additional address page. I
have accounts with several locations. I have created my form in VBS using a
MultiPage. I would like to add a command button that will add a page to the
multipage when clicked. I intend on deleteing the second page when the
Multipage is created so the form shows one tab. It would be perfect if I
could have the name of each tab be 01,02,03 etc. I would also like to have a
command button in my contacs form to take me to this form and be sure that
the info is tied with that contact.

That's not going to work. You would have to add not just another multipage, but also controls on that page. But adding controls at runtime one-offs a form so that it will not run code again. I think you're going to have to rethink your design.
