Default Write a Post-Termination report on project

I have just finished a project to check into moving our Outlook Forms from
the Organization FOrms Library to a Public Folder. I completed the testing
using differenct folders, permissions and testing to see how they would work
and listing all the pros and cons.

Now I need to write a Post-Terminatin Report detailing all our findings and
recommendations. Does anybody know where I can fine samples or instructions
on how to do this?
