I have created a form and when I publish it to my personal folder it looks
ok. However, when I send the form all that shows up is the To: From: and subject field. The whole body is blank |

A published message form will work only if (a) the "send form definition with item" box is unchecked on the Properties page and (b) each user has access to the published form definition.
