Default Redesigning a Contact Form

I have redesigned my contact form and added another space for a phone number
along with the drop down menu. When I added the "Title" and Empty box, now I
need to add the drop down button - i can't seem to find it to add so I just
copied the one above it - but this associates it with the one above. How do
I associate it with my new line?

Default Redesigning a Contact Form

Look in the Field Chooser, under All Contact fields. You'll see that there are up to 8 independent phone selectors you can use to hook up with the built-in phone fields.
