Default Unchecked Check Boxes

Using Outlook 2003 and a modified Message Form from the Standards Forms
Library, I've published the form to the Personal Forms Library. The modified
form contains a series of check boxes. When I check some or all of them, and
send a form to myself, the form arrives all unchecked.

Default Unchecked Check Boxes

Sounds like you did not create a yes/no field for each check box and associate each box with a field, using the control's Properties dialog, on the Value tab.
