How do I use an existing Office 2003 form as a template in Outlook? This is
just for our office environment.

In general, that's not possible, but without details, it's hard to suggest an alternative.

Our office has many forms they complete, then the staff member has to type an
email with much of the same information to send to other staff members. It
would be more efficient to have various templates set up in Outlook - which
have been previously created in Office 2003 - in order for the staff member
to complete the template in the same format each time and easier to do so
because of checkboxes, etc. Is this enough information?

Not quite. You didn't say anything about your mail environment. The only way to get message forms to work is if you can publish them to the Organizational Forms library on an Exchange server or if you can get each user to publish each form to their own Personal Forms library. Even if you meet that requirement, an Outlook message form would have to be designed from scratch, not from an existing Word form, for example.
