Default Outlook Template

I need to make a template that can be filled out by out help desk, sent to
the customer for them to complete, then sent back to our help desk. I
created a template using outlook and I could not see how to lock the form
down. Is this even possible? I created a template in word using tables but
I don't see a way to get that into Outlook. Any suggestions? I am stumped.
