Default How do I start a Form?

I am looking for a way to start a form I created in VB editor via either a
custom action or script. I have no idea how to do either.

So can anyone help me figure out how to open a form from an action or rule.

Also, How do I save a script or custom action for use in a rule?

Please and thankyou,

Brian
