How do you delete a customized form in Outlook?

I was just playing around in Outlook--learning the advanced features--and
created a new contacts form. How do I delete it now? I've tried File/Delete
but it keeps reappearing in both my contacts folder and my personal forms
folder.

You pull up the properties panel of any folder, click the Forms tab and use the
forms manager control to find the form definition and delete it through the
forms manager. In some of the older versions of Outlook it is somewhat more
difficult to find the forms manager.
