Default how to add control similar to 'contact button'

Anyone have any thoughts on this?

Also, is it possible to have a drop down control that pulls its possible
entries and limits them to cells on an excel worksheet? I'm not a 'coder' so
any direction to possible links for code to do this, or an example would be
dearly appreciated.
