Default Creating fields to be seen in any email setup.

I'm trying to create a form that has a Project Name and Project Number with
boxes. I have one set up but it is only able to be viewed through Outlook
2003. I'm needing to be able to customize it so no matter who I send it to
they are able to see the Project Name and Number with the information given
in the sent email. How can I accomplish this?
