Where do I get form templates for Task or Calendar in Outlook?

Default Where do I get form templates for Task or Calendar in Outlook?

Hi I'm looking for a form (Task or Calendar, what ever is the best) in
Outlook 2000 and higher, to help me in my appointements follow-up and
reporting with my boss. The field that my boss is asking are The full contact
(Full name, Compagny & Function), Time and Date of the phone call, the where
field if it was more an appointement instead of a phone call, the date & time
of reminder as the follow-up date & time field. It will be a nice to have if
this field can pop-up in Outlook.
Also, if this template could be modify it will be a nice wish to have. I
don't want to go with Symantec ACT! like my boss says. I'm sure OUTLOOK can
do the job and more.

Did you check out Business Contact Manager for Outlook? I think it may do
what you are looking for and is a free add-on from Microsoft. You should be
able to find it in a download search.
