Default From Field Not Populated

I created a form and added a From text field on th read page. However, when I
run the form the From field is empty. I've even copy and pasted this field
from other forms where it works and made sure all the seetings are the same
and it still doesn't work.

Are you looking at messages you've sent with this form that are in your own Sent Items folder?
