Default Custom Outlook Form is not visible to reader????

Hi - I have created a custom message form in Outlook 2007. I have published
it to a company wide folder, however, when filled out and sent to myself, it
does not pull up the form -- only a blank email.

When I am in design mode and click 'run this code' it works beautifully, but
not when published.

Any ideas? Many thanks!
