Default How to remove Published forms?

I was running MSOutlook 2007. I created a form and published it. I accessed
it via Actions tab. I select it, fill it out and send it out.
I uninstalled Outlook 2007 and installed Outlook 2003 and the form is still
under Actions. I can select it, fill it out and Send it, but it gets stuck in
Outbox and does not get Sent. I would like to remove it from the Actions tab
and reinstall it. How do I remove a Published form ?
