Default Route Form in Outlook 2003

I'm trying to create a new form that will allow me to send a message
sequentially to a list of Outlook recipients...

I've tried this form:

http://www.outlookcode.com/d/forms/routemsg.htm

but it does not seem to work with 2003...the route button works but all it
does is open up a new form for me to manually input the recipients again...

any suggestions?
