Default Forward a message in a custom form

thank you for trying to help me.
we have public folder which recieves e-mails with attachments from our
customers, ist it possible to forward each recieved e-mail to another public
folder but when forwarding we need to use an outlook form (in this form are
some fields which will be filled up) without losing the original e-mail or
attachments?
any ideas?

thx
