modify outlook form-appointment form

Default modify outlook form-appointment form

I'm interested at adding some controls in the standard appointment form.
But when I open the appointment form and chose "design this form", I can't
modify anything in the first slide "appointment". so how to add some controls
in the "appointment" slide?
