The settings I entered were actually correct and complete but there might
have been a registry error which was not removed by my entering the settings.
Only after I added a location and *removed*...

1. United States, valid area code, no special dialing rules, using tone.
2. Area code rules: use 1+ area code for all calls.
3. Calling cards: None.

Sorry if I wasn't clear enough. I'm quite sure that before I reinstalled the
PC, when I entered 2125551212 and tabbed to the next field the tel number was
shown as (212) 555-1212. Now it remains...
